ILLINOIS PHARMACISTS NOW AUTHORIZED TO PROVIDE HIV PREVENTION MEDICATION WITHOUT PRESCRIPTION

The Illinois Department of Public Health (IDPH) has announced a new initiative to improve access to HIV prevention through a standing order allowing pharmacists across the state to dispense PrEP (pre-exposure prophylaxis) directly to individuals at risk for HIV, without the need for a prior doctor’s prescription.

 

PrEP is widely recognized as one of the most effective tools in preventing HIV transmission, reducing the risk by up to 99 percent when taken as directed. The new order, signed by IDPH Director Dr. Sameer Vohra, aims to make this critical medication more accessible, especially to populations disproportionately affected by HIV.

 

“Expanding access to PrEP through pharmacists is a significant step forward in our efforts to reduce new HIV infections in Illinois,” said Dr. Vohra. “This initiative, combined with our ongoing PrEP4IL program, helps us reach more individuals at highest risk and move closer to our goal of zero new HIV transmissions by 2030.”

 

The order streamlines the process for individuals to obtain PrEP, which is typically available as a daily oral pill regimen. It also facilitates reimbursement for pharmacists providing these services. Under Illinois law, pharmacists must complete specialized training to offer PrEP services, including guidance on testing, confidentiality, and record-keeping.

 

HIV remains a concern in Illinois, although progress has been made. In 2024, there were 1,386 new HIV diagnoses statewide—a nine percent decrease from the previous year. However, Black and Hispanic communities continue to be disproportionately impacted, accounting for over 60% of new infections annually.

 

The Illinois PrEP4IL program offers medication assistance to reduce financial barriers for those seeking PrEP. Last year, over 900 applications were received, with significant growth among Black and Hispanic populations.

 

Individuals at risk for HIV—such as those with an HIV-positive partner, injection drug use, inconsistent condom use, recent STI diagnosis, or sharing needles—are encouraged to speak with their healthcare providers about PrEP.
